Health Requirements for BMCS Students

​

• Immunization Record: Must be completed by the first day of school. Submit a signed exemption letter if applicable.

• ​

• Health History Form: Return completed form no later than the first day of school. Include the immunizations/tests section or attach a list.
   

• Physical & Dental Forms: Required in the first year. Have exams done by your family doctor and dentist. Return signed forms no later than the first day of school.​

​

• Medication: A signed form is needed for all medications given at school. Prescriptions require a doctor’s signature.

​

• Health Updates: Inform the office of any serious illness, accidents, or health issues during the year.
